This church is built over a ditch of the old Roman city, Melite which included Mdina and a large part of Rabat. It is also built over the grotto where St Paul was allegedly kept prisoner during his stay in Malta.
In 1336, Bishop Hilarius referred to the church as "ecclesia Sancti Pauli de crypta", and also mentions the cemetery and the Roman ditch.
Several churches had been built on the site, both by the diocese and by the Knights of St John. The present church was built with funds provided by the noblewoman Guzmana Navarra on plans prepared by F. Bonamico. It was completed by Lorenzo Gafa' in 1683.
